Complex radiodiagnosis of kidney angiomyolipoma
Objective: To define the capabilities of ultrasound tomography, computed tomography, helical CT (HCT), magnetic resonance imaging (MRI) and intravenous urography (IVU) in detecting and differential diagnosis of kidney angiomyolipoma (AML).
Material and Methods: The results of treatment of 158 patients aged 16-86 (128 women and 30 men) with kidney AML were analyzed. Complex clinical laboratory examination, traditional UST, IVU and additional MRI, CT and HCT were performed.
Results: Complex rediodiagnosis of AML was determined to depend on the tumor histology, size, location, the volume of the examination and thorough data analysis.
Conclusion: UST is an effective method for kidney AML diagnosis (sensitivity - 99.3 %, specificity and accuracy about 100 %) as the picture of AML is typical. MRI of AML depends on the fat amount in the tumor, which is seen as high signal intensity area in the parenchyma in MSSE T1 sequence (sensitivity 98-100 %)/ when vascular or muscular components prevail and the size of the lesion is small. AML can be iso-intensive with the kidney parenchyma or is not seen even after contrast substance administration. CT picture of AML also depends on the tumor morphology. When fat prevails in AML , CT without contrast enchancement is characterized by a lesion with a dense fat tissue without its enchancement after a contrast administration (sensitivity 98-100 %). But when vascular and muscular components previal, the fat area is small, the sections are thicher than the fat inclusions, AML can be missed. Small sign-free AML with a typical US picture can be detected with UST in dynamics, when the tumor size is up to 4 cm once a year or two years, when the size is > 4 cm, once in 6-12 month. CT (HCT), MRI are indicated for final AML diagnosis. IVU is not effective in diagnosis of AML < 4 cm; they are observed when the size is > 4 cm.

Calcifications in the epiphysObjective: To study the incidence of calcification formation in the epiphysis and vascular plexuses (VP) of the lateral ventricles (LV) in stroke, one of the commonest brain diseases.
Material and Methods: The study was done by means of analysis of archival brain CT scans of 170 patients aged 22-74, of them 46 with transitory ischemic attacks (TIA) (from 3 to 12 days after the episode), 57 with hemorrhagic stroke (from 3 to 8 days), 67 with ischemic stroke (1-4 days after the episode). The controls were diagnostic images of 53 healthy persons without the history of brain or ENT diseases and operations. All findings were processed with the use of Microsoft Excel 97.
Results: The findings of the study demonstrated that the incidence of calcifications in the epiphysis and LV VP in TIA patients was 28.3 and 17.4 %, HS - 42.1 % and 24.6 %, IS - 56.7 % and 40.3 %, respectively . The values for HS and IS patients significantly differed from those in the controls (P<0.05), 26.4 % and 11.3 %. Calcifications in the epiphysis and LV VP are labile structures capable of diminishing. Repeated examination of 8 patients after HS (group 1) and 15 patients after IS (group 2) 8-11 months after the first examination showed considerable diminishing of calcifications in 3 patients of group 1 and 6 patients of group 2. These patients demonstrated complete restoration of the neurological state.
Conclusion: The incidence of calcification formation in the epiphysis and vascular plexus of the lateral ventricles in patients with hemorrhagic or ischemic strike increases when compared with healthy persons . Elimination of neurological deficiency in the patients is accompanied by calcification diminishing. The analysis of archival brain CT scans of the patients with various diseases of the head and neck can promote eliciting clinical benefit of calcification description in the epiphysis and vascular plexus of the lateral ventricles as well as other locations ( basal ganglia , falx of the brain ).

Diagnostic capabilities of ultrasound study in stenosis of the spinal canal lumbar portion
Objective: To study the capabilities of ultrasound diagnosis in stenosis of the spinal canal lumbar portion, to work out standardization of qualitative and quantitative parameters of the method.
Material and Methods: Ultrasound study was done in 51 patients (240 disks aged 23-58 (of them 27 men and 24 women, 90 disks) with a clinical diagnosis of lumbar osteochondrosis. The controls were 18 persons aged 23-41 without clinical and instrumental signs of this disease. The findings were compared with the surgical data (8 operations), the findings of x-ray study, CT, MRI. B-mode USS was done through the transabdominal approach using Aloka SSD-630, Radmir P20, Myson (Medison) scanners with a convex 3.5 MHz probe. Staged axial slices from L1-L2 to L5-S1 allowed to obtain the image of the disk and spinal canal (SC) similar to CT but at the level of mobile segments. The sagittal SC size and its area were measured using a planimetric technique with the account of the state of yellow ligaments and hernia of the intervertebral disk.
Results: In the majority of cases (24 patients) stenosis of the spinal canal was acquired due to degenerative changes in the disks. Considerable diminishing of all lumbar vertebrae without hernias, i.e. diaplastic stenosis, was revealed in 2 patients, dislocation stenosis was present in 4 patients. Of 28 persons with stenosis, 8 were operated on. Of them, 4 had traumatic disk rupture, the other 4 - hernias causing severe stenosis. Median and paramedian hernias were clinically significant. Concentric stenosis of the SC was diagnosed in 2 patients.
Conclusion: The performed investigations showed that ultrasound study allows easy and accurate calculation of the area of the spinal canal at the level of the disk as well as takes into consideration the characteristics of the hernia localization and its influence on the size of stenosis, thus aiding in correct assessment of the signs and choosing the most optimum method of treatment.

Modern x-ray manifestations of tuberculous spondylitis
Objective: To improve the efficacy of diagnosis of tuberculous spondylitis.
Material and Methods: The findings of traditional x-ray examination of 175 adult patients with active newly revealed tuberculous spondylitis treated in Regional Tuberculosis Hospital No. 1 (Kharkiv) during 1990-2003 were analyzed.
Results: The study allowed to reveal modern characteristic x-ray manifestations of the disease, i.e. depth and form of destruction of the vertebral bodies, sclerotic reaction of the bone tissue, localization of destructive foci in the vertebral bodies, spine deformity.
Conclusion: Traditional x-ray study is the basic method of the spine investigation which allows establishing the features of the bone structure, location, character and volume of the destruction of the vertebral bodies, sequestrations, interrelation of the destruction cavities. CT and MRI should be primarily used when diagnosing isolated tuberculosis ostitis or accompanying distant foci.

Doppler echocardiography of transmitral blood flow in diagnosis of diastolic dysfunction at chronic heart failure progress
Objective: To estimate the changes of mitral blood flow parameters as markers of diastolic functions of the myocardium with the help of Doppler echocardiography at increase of heart failure severity.
Materials and Methods: 84 patients with class II-III HF were examined. The controls included 20 healthy persons. The diastolic functions of the left ventricle ( LV ) was examined using Doppler echocardiography in a pulsed mode, with a unit Toshiba SSH-160A ( Japan ) with a 3,5 MHz probe.
Results: The carried out analysis showed that if in patients with class II CHF dominating in pathogenesis is the hypertrophy of LV myocardium, in patients with class III HF -systolic dysfunction. As the main pathogenetic factor in increase of HF severity from class II to class III is diastolic dysfunction.
Conclusion: In patients with class II HF, when the left atrium is intact, diastolic mitral blood flow abnormalities of "slow relaxation" type occur. In patients with class II against HF II, a background of the increased sizes of the left atrium, cardiosclerosis and arrhythmia, are diagnosed as «pseudo-normal» type of mitral blood flow. For the majority of patients with class III HF, "restrictive" type of mitral blood flow is diagnosed.

The peculiarities of x-ray and ultrasound pictures in various breast cancer histology
Objective: To study the characteristic x-ray and ultrasound signs of various histological forms of breast cancer (BC), especially minor cancer (MC).
Material and Methods: The study involved 120 patients with BC aged 27-68. Ultrasonography was done using ЭТС-У-02 unit with a 7 MHz probe with a water attachment and Siemens G-50 unit with a 7 MHz probe. Ultrasound guided puncture biopsy was performed. X-ray study was done using Electronica mammography unit. The tumor histology was determined according to the WHO classification No. 2, 1981, Russian variant 1984.
Results: Two main forms of BC growth, nodular and diffuse, were distinguished. In nodular forms mammography (MG) demonstrated a shadow of a delimited node, in diffuse a distinct shadow was absent.
X-ray and ultrasonographic pictures of various histological forms were different in spite of an apparent similarity.
On MG, scirrhus looked like a star-like formation with a dense centre, bands in the gland tissue with frequent small calcifications, while ultrasonography demonstrated an echo-positive shadow with indistinct strange outlines.
In adenocarcinoma, MG showed an ameba-like node with short bands, the structure was changed only near the tumor calcifications were absent. Ultrasonography demonstrated round homogeneous, as a rule echo-positive formations with wavy indistinct outlines.
Solid cancer looked like a nodular round shadow with indistinct wavy outlines, radial bands in the surrounding tissue on MG, on ultrasound scans it looked like an echo-negative shadow with wavy outlines. Inhomogeneous structure was frequently due to hypo- and hyperechoic inclusions.
In medullar cancer MG demonstrated a distinct oval or lobular, node more distinctly delimited than in scirrhus or solid cancer; ultrasound showed a distinct nodular shadow with the signs of homogenicity, frequently with increased dorsal echo.
On MG lobular invasive carcinoma was characterized by changes in the structure without a visible tumor mass; on ultrasound scans it was characterize by hypoechoic changes with disseminated dark foci.
Conclusion: Complex use of radiodiagnosis techniques provides timely diagnosis of early forms of cancer. This allows to suggest the tumor histology before the operation. X-ray and ultrasound pictures are characterized by their own peculiarities depending on the tumor histology. Ductography is a highly informative method revealing intraductal breast tumors.

Ultrasonic diagnosis of heart membranes disorders in children
Objective: To develop quantitative criteria for diagnosis of ultrasonic density of heart membranes in children for determination of the changes and outcomes of heart inflammatory diseases.
Material and Methods: On the basis of the original method, the data of one- and two-dimensional echocardiography were used to diagnose ultrasonic density of heart membranes in 46 healthy and 65 children aged of 1-16 with non-rheumatic myocarditis.
Results: Referens parameters of ultrasonic density of endo-, myo- and pericardium in healthy children were developed. Measurement of ultrasonic density in the standard zones of heart membranes using computer processing of echocardiograms allowed to diagnose the deviations associated with inflammation in non-rheumatic myocarditis. Frequency and degree of heart membrane involvement in the pathological process were determined in myocarditis, which depended from on the disease severity.
Conclusion: Application of the method of quantitative diagnosis of ultrasonic density of the heart membranes and their structures allows to increase objectivity of final diagnosis in determining, prevalence and degree of involving endo-, myo-and pericardium by inflammatory process in children with myocarditis. Non-rheumatic myocarditis is characterized by increase of ultrasonic density of structures of myocardium and endocardium of the interventricular septum and posterior wall of the left ventricle.The most expressed are significant and acute changes in the myocardium of the left ventricle. Pathological deviations of ultrasonic density of pericardium of right and left ventricles in children with non-rheumatic myocarditis are registered in solitary cases.

The criteria for diagnosis of proliferative inflammatory changes in the mucous membrane of the maxillary sinuses using helical CT findings
Objective: To study the posibility of diagnosis of proliferative (irreversible) inflammatory changes in the mucous membrane of maxillary sinuses using helical CT findings.
Material and Methods: CT scans of 32 patients (15 women and 17 men) aged 11-66 (main group) with inflammatory and inflammatory-proliferative diseases of the maxillary sinuses who were performed HCT (the study of the nasal and paranasal sinuses) during the exacerbation of the inflammatory process as well as during remission were analyzed. The interval between the first and repeated HCT study was 10 days - 34 months. Dynamics of the inflammatory changes was studied in the main group with the purpose to select the patients with reversible changes in the mucous membrane of the maxillary sinuses. Reversibility of the pathological process was evaluated as to normalization of the mucous membrane thickness and absence of clinical manifestations of maxillary sinusitis.
The controls were 10 patients (5 men and 5 women) aged 11-53 with chronic inflammatory-proliferative maxillary sinusitis who were performed operative treatment with removal of the changed mucosa and histology verification of the inflammatory-proliferative process (irreversible changes) following HCT.
Results: X-ray density in the areas with polypous-fibrous changes in the mucous membrane was higher (up to 78.8 HU; CO = 12.0 HU), than in the areas with polypous and cystic-polypous changes (up to 54.0 HU; CO = 21.5 HU).
The indices of mean x-ray density in various areas of the thickened membrane differed considerably even on one CT scan.
The parameters of mean x-ray density of the areas of reversible thickening may have similar values when compared with the areas of irreversible (proliferative) thickening of the mucous membrane of maxillary sinuses.
Conclusion: HCT is reasonable in diagnosis of chronic inflammatory proliferative changes in the mucous membrane of paranasal sinuses during remission of the inflammatory processes. In the acute phase the risk of a diagnostic errors increases.
The parameters of x-ray density of the thickened mucous membrane of maxillary sinuses cannot be a significant criterion for differential diagnosis of reversible and irreversible (proliferative) inflammatory changes in the mucous membrane.

Increase of radionuclide ventriculography informativity in evaluation of contractile function of the left ventricle in patients after myocardial infarction
Objective: To determine the informativity of end-systolic volume (ESV) to stroke volume (SV) ratio as a criterion for evaluation of contractile myocardium function in patients after myocardial infarction (MI).
Material and Methods: The findings of radionuclide ventriculography ( 99 mTc-pyrophosphate, indicator dose 370-450 MBq) obtained from 107 patients with MI and 22 healthy volunteers were analyzed. Ejection fraction, end diastolic volume (EDV), ESV, SV were determined; ESF/SV, so-called contractile function index (CFI), was calculated. CFI sensitivity, specificity, prognostic significance in respect to positive and negative results suggested CIF informativity in evaluation of contractile myocardium activity.
Results: Reduced EF and increased CFI were observed in patients after MI. In group 1 (normal EF) CFI was within the range of normal values determined for this parameter. With worsening of LV function (reduction of EF), statistically significant increase of CFI was observed. The degree of CFI increase was higher than the degree of EF reduction. In group 2, EF reduction was 20.7%, CFI increase - 38.6%, in group 3 -36.5 and 105.3%; in group 4 - 46.1 and 210.5%, respectively.
Comparison of informativity criteria for the two parameters revealed increased CFI sensitivity in evaluation of myocardium contractile function by 21.9% when compared with EF, high specificity (90.9 and 95.4% for EF and CFI, respectively) as well as prognostic significance of a positive finding (90.9 and 95.4% respectively) and increase of a negative result by 13.6% (58.8 and 72.4%, respectively for EF and CFI).
Conclusion: The use of CFI allows to evaluate myocardium contractile function more precisely when compared with EF and analyze possible mechanisms of heart adaptation to the changes in cardiodynamics due to MI.

The influence of polarized light on prooxidant-antioxidant homeostasis in children frequently suffering from acute respiratory infections
Objective: To study the influence of PILER light on the dynamics of prooxidant-antioxidant homeostasis in children frequently suffering from acute respiratory infections depending on the type of adaptation reactions.
Material and Methods: In 179 children frequently suffering from acute respiratory infections and 90 children, who suffer from acute respiratory infections episodically, aged 3-11, peripheral blood leukogram was used to determine the type of general nonspecific adaptation reaction (GNAR). The amount of lipid hyperperoxides in the blood serum was calculated according to the equivalent amount of malonic dialdehyde. Glutathione peroxidase activity was determined using spectrophotometry; antioxidant serum activity was studied according to its capability to inhibit thiobarbituric-acid-active products of lipid peroxidation. Preventive course of PILER light therapy was administered to 46 children frequently suffering from acute respiratory infections in an individual mode. The dynamics of pro-antioxidant homeostasis was determined according to the indices of pro-antioxidant balance (IPAB).
Results: Heterogenicity of adaptation-reserve capabilities was revealed in children, which was confirmed by the incidence of polymorbid states, resistance level, shift of homeostatic balance to pro- and antioxidant processes. The influence of PILER light on the dynamics of pro-/antioxidant homeostasis depended on the type of GNAR: in stress reaction pro-/anti-oxidant processes were balanced, in training reaction the processes of peroxidation activated, in quiet activation IPAB parameters preserved; in increased activation, the balance shifted to the side of antioxidant protection; in reactivation, a tendency to normal pro-/antioxidant balance was observed.
Conclusion: The method of integral evaluation of the state of pro-antioxidant homeostasis with calculation of IPAB and preliminary determining the type of adaptation reaction allows to substantiate administration of antioxidant drugs and can be used as a criterion of PILER light therapy efficacy in children.

The peculiarities of treatment-related pathomorphosis of invasive cervical cancer at simultaneous use of radiotherapy and fluoropyrimidines
Objective: To determine the efficacy of radiation therapy (RT) in combination with 5-fluorouracil (5-FU) or capecitabine in patients with cervical cancer (CC).
Material and Methods: The multimodality treatment was administered to 140 patients with T 1B 2 B N0 1M0 CC. Comparative analysis of the treatment pathomorphosis of the distant tumors was done.
Results: Radiation pathomorphosis was more pronounced after fluoropyrimidines administration and was 16.4 ± 1.3 (combination of RT and 5-FU) and 8.1 ± 0.8 (combination of RT and capecitabine) vs 40.5 ± 3.4 (after RT) and 8.5 ± 6.2 (without RT and its modification).
Conclusion: The analysis of treatment pathomorphosis of cervical carcinoma demonstrates different anti-tumor efficacy of the administered modalities of pre-operative antineoblastoma therapy. Comparison of the results allows to conclude about relative advantages of fluoropyrimidines administration, namely xeloda, in radiomodifying doses during pre-operative radiation therapy for disseminated CC.

Mathematic simulation of the processes determining survaval curve of exposed eukaryotic cells
Objective: To build mathematical models of the processes determining the shape of survival curves characteristic for various cells, which have a typical “shoulder” at low doses and a linear portion at high doses.
Material and Methods: Probability simulation, algorithmi-zation, programming.
Results: The created models use modern ideas about a dominating role of reparation processes in forming survival curves with the account of a known suggestion about existence of saturation of cellular reparation systems. But in contrast to the known approaches to the saturation phenomenon, the created models are based on a clear definition of the essence of this phenomenon. Besides, the described models take into consideration the association of structural-functional features of cellular chromatin and survival of the exposed cells. The findings of the research can promote better understanding some radiobiological processes.
Conclusion: The models presented in this work allow a new interpretation of the known phenomena of existing sublethal and potentially lethal lesions, which, in our opinion, lacks some contradictions characteristic to the existing models.

The study of skin ultrastructure in patients with breast cancer after local fractionated irradiation of the tumor
Objective: To determine ultrastructural borders of radiation pathomorphism in the breast skin for substantiation of the choice of surgical resection at modified subcutaneous mastectomy with primary reconstruction by the rectoabdominal flap in breast cancer patients.
Material and Methods: Standard methods of electron microscopy were used to study the breast skin ultrastructure on the 1 st day after local fractionated gamma-therapy at a total dose of 25 Gy.
Results: It was established that after irradiation vacuolization of the cytoplasm, nuclear pyknosis, swelling of mitochondria in the basal keratinocytes and cells of the of the spinous layer located over the basal cells, disturbance of intracellular contacts up to their rupture, disappearing of the basal membrane, detachment of the epidermis from derma, its infiltration with lymphocytes and macrophages, radiation pathomorphism of the capillaries take place in the skin. The degree of these changes decreases from the center of the radiation field to its margins with disappearing outside 10% isodose curve.
Conclusion: Electron-microscopy criteria for radiation lesions of the epidermis and derma of the breast skin in the conditions of local gamma-therapy were established. Dose dependence of skin radiation pathomorphism in accordance with the characteristics of the radiation field was revealed. To determine the margins of irradiated skin resection at surgical reconstruction of the breast, the characteristics of the radiation field can be used; this can be done behind the 10% isodose curve.

Multi-factor analysis of combined action of ionising radiation and cadmium ions on the organism
Objective: To evaluate combined action of ionising radiation and cadmium ions on the organism using the method of multi-factor analysis.
Materials and Methods: Experimental animals (male rats) were exposed to x-rays (at dose of 1.0Gy or 2.0Gy, dose rate 0.35 Gy/min), combined or separately with cadmium chloride influence (1.0 mg/kg of body mass in conversion to Cd 2+). The subjects of investigation were the liver and small intestine (duodenum and jejunum). Lipid content, enzyme activities, lipid peroxidation products and cytomorphometric indices were investigated. The analysis of received array of biological indices was represented as linear combination of several hypothetical variables - factors or principal components. They reflect the existent correlation between variables. The effect of combined action of external factors was evaluated based on principal factors from all measuring indices. The synergism coefficient was also determined.
Results: The difference in biological status of the organisms exposed to separate or combined action of ionising irradiation and cadmium ions was revealed. The analysis of obtained data testifies the different mechanisms on the basis of separate and combined action of ionising irradiation and cadmium ions. The observed changes had nonadditive and non-monotone character. The competition of action effects was also revealed that appeared in observed decrease of synergism coefficient.
Conclusion: The expediency of use of factorial analysis on the basis of biological indices measuring for estimation of separate and combined action of physical and chemical agents on the organism is shown.
The indices allowing to evaluate the effect of combined action of ionising radiation and cadmium ions in experimental conditions, are revealed.
The synergism coefficient that testifies the competition between the effects of combined action of ionising irradiation and cadmium ions is determined.

The role of radiation therapy in treatment of liver metastases
Objective: To work out and introduce to the clinical practice a program of complex treatment for metastatic lesions to the liver as well as to study immediate and long-term results of this treatment.
Material and Methods: Thirty-two patients suffering from various forms of cancer accompanied by liver metastases were administered radiation therapy with dose superfractionation 2 times a day with a single focal dose of 1 Gy every 4 hours.
Results: Pain disappeared in 12 patients in the middle of the course of treatment and in 20 patients by the end of the course. In 26 patients biochemical indices began to normalize.
Conclusion: In 82% of cancer patients with liver metastases, radiation therapy produced a pronounced symptomatic effect including correlation of biochemical indices.
The suggested mode of the liver irradiation does not produce radiation reactions and complications associated with radiation lesion of the liver.
Distant radiation therapy is a simple and accessible technique for treatment of liver metastases. It can be used even in disorders of the liver function, i.e. when chemotherapy is contraindicated.
